Personal Data Application Form

Pursuant to the Law No. 6698 on the Protection of Personal Data (“KVK Law”), personal data owners defined as the “relevant person” (“Hereinafter referred to as the ‘Applicant’”) are granted the right to make certain requests regarding the processing of their personal data under Article 11 of the KVK Law.

Pursuant to the first paragraph of Article 13 of the KVK Law; applications to be made by the Applicant regarding these rights must be submitted to our Company, which is the data controller, in writing or by other methods determined by the Personal Data Protection Board (“Board”).

Within this framework, applications to be made to our Company “in writing” may be submitted by taking a printout of this form;

• By the Applicant’s personal application in person,
• Through a notary public,
• By being signed with a “secure electronic signature” as defined in the Electronic Signature Law No. 5070 by the Applicant and sent to the Company’s registered electronic mail address.

Your applications submitted to us shall be responded to within thirty days from the date your request reaches us, depending on the nature of the request, pursuant to paragraph 2 of Article 13 of the KVK Law. Our responses shall be delivered to you in written or electronic form pursuant to Article 13 of the relevant KVK Law.

(Your application will be finalized free of charge; however, if the transaction requires an additional cost, the fee specified in the tariff determined by the Board may be charged.)

This application form has been prepared in order to identify your relationship with our Company and, if any, to fully determine your personal data processed by our Company, and to respond to your relevant application accurately and within the legal period.

In order to eliminate legal risks that may arise from unlawful and unjust data sharing and especially to ensure the security of your personal data, our Company reserves the right to request additional documents and information (copy of identity card or driver’s license, etc.) for identity and authorization verification. In case the information regarding your requests submitted within the scope of the form is not accurate and up to date or an unauthorized application is made, our Company does not accept responsibility for requests arising from such incorrect information or unauthorized application.

If the transaction to be carried out pursuant to your request requires an additional cost, a fee may be charged as stated in the Communiqué on the Procedures and Principles of Application to the Data Controller. A processing fee of 1 Turkish Lira may be charged for each page over ten pages. If the response to your application is required to be given on a recording medium such as CD or flash drive, the fee corresponding to the cost of such recording medium may be requested from you.

In line with the requests stated above, I kindly request that my application submitted to your Company be evaluated pursuant to Article 13 of the Law and that I be informed. I declare and undertake that the documents and information I have provided to you in this application are accurate, up to date and belong to me.

I consent to the processing of the information and documents I have provided in this application form by your Company, limited to the purposes of evaluating and responding to my application made pursuant to Article 13 of the Law No. 6698 on the Protection of Personal Data, delivering the response to me, and determining my identity and address.
